A My City My Health initiative

The Checkpoint.

A community-powered platform helping Black men understand prostate cancer risk, take action, and make their health stories count.

Prostate cancer affects Black men at higher rates. But awareness alone is not enough. Save the Homies connects men to trusted information, free PSA kit access, local resources, and a community health survey that helps us understand what men need to move from awareness to action.

Community stories. Prostate cancer data. Action for the homies.

Built by Save the Homies, a My City My Health initiative, to connect community voice, cancer data, and real action.

Why this matters

This is about more than awareness.

Black men face a higher burden of prostate cancer, yet many men do not get the information, trust, access, or follow-up they need. Save the Homies exists to close that gap through storytelling, community education, and action.

Know the Risk

Learn why prostate cancer screening conversations matter, especially for Black men and men with a family history.

Take Action

Eligible men can request a free PSA kit and start the conversation about their health from home.

Shape the Data

Your voice helps us understand barriers, trust, access, and what men need from health systems and community partners.

The Checkpoint

Prostate cancer data tells part of the story.

Official cancer data helps us understand long-term trends. But community insight helps us understand what men are experiencing right now. The Checkpoint brings both together.

Iowa Burden Prostate cancer cases & deaths per year in Iowa
Black Men's Risk Diagnosis and mortality rates for Black men nationally
Screening Awareness Share of men who've talked with a doctor about PSA
Community Trust Who men trust most for health information
Barriers to Care Top reported reasons men aren't getting checked
PSA Kit Requests Free kits ordered through Save the Homies
ZIP Codes Reached Iowa communities engaged through the platform
Stories Collected Survivor, family & community voices in the index

Data coming soon

Data coming soon from public cancer data sources and Save the Homies community responses.

This platform shares educational information and community insights. It does not provide medical advice. Men should talk with a healthcare provider about prostate cancer screening and follow-up.
Get Checked

Start with one step.

Getting checked does not have to start with a clinic visit. Eligible men can request a free PSA kit sent to their home.

Questions to ask your doctor

  • Should I be screened for prostate cancer?
  • Does my family history change when I should start?
  • What does a PSA test measure?
  • What happens if my PSA result is high?
  • How often should I get checked?
Community Survey

Take The Checkpoint

This short survey helps Save the Homies understand what Black men know, need, and trust when it comes to prostate cancer and preventive health. Your answers help shape future outreach, resources, events, and community reports.

Age range *
Have you ever talked with a doctor about prostate cancer? *
Have you ever had a PSA test? *
Do you currently have a primary care provider? *
What is the biggest barrier that keeps men from getting checked? *
Who do you trust most for health information? *
Would you like resources or reminders from Save the Homies? *

Optional — only if you want to share.

Your responses help us understand community needs. We will not sell your information. Public reports will only use grouped data, not personal answers.
Share Your Story

Your story could help another man take action.

Some people want to write their story. Some people would rather say it out loud. The Checkpoint gives men, families, survivors, caregivers, and community partners more than one way to share what prostate cancer awareness, screening, diagnosis, treatment, or loss has meant in their lives.
